StreetLife Promotions Event: Waiver and Release Agreement (please download application at the end of this page)

Note: References of the “I” in the proceeding waiver and release agreement refers to participants, their families, persons or any entity, etc that has a security interest in participants.

By registering for the (title of event) in (country) where StreetLife Promotions or any of its affiliated companies are promoting or hosting action-sporting events, I am acknowledging my understanding that operating a (motorcycle, automobile or bicycle) is an activity that contains considerable risk of serious personal injury, paralysis, dismemberment, property damage or loss or death.

I am of sound mind and body and of legal age to sign this document. I, hereby knowingly and freely accept risks of all personal injury, property damage arising from my participation in these types of action-sports entertainment.  I hereby agree, that I, my dependents, heirs, successors, personal representatives and assigns do release and hold harmless against, and indemnify StreetLife Promotions (the promoter/host), and all sponsors, its owners, employees, and agents from any and all claims, including claims of death, bodily injury, illness, property damage or loss, however caused, arising from or related to the (title of event) events  or my participation in it, even if arising from the negligence of the releases or others.

I agree to ride or drive within my riding or driving abilities and agree not to ride or drive while impaired by the influence of alcohol or drugs, and to not be influenced by any group or pack mentality to ride or drive faster than is within my ability or the limits of what is considered safe riding or driving conditions. Nor will I be influenced by the riding or driving displays by my fellow rider or driver participants that could be considered unsafe.  I fully understand that I may encounter riding or driving conditions and roads that I have not traveled before, and have no prior knowledge of, and therefore will ride or drive within a manner that is considered safe for those riding or driving conditions. Furthermore, I agree that I will not in any circumstance ride (a motorcycle or a bicycle) or drive an automobile, be it my own, or a rental (motorcycle, bicycle or automobile) in the (title of event), beyond what is commonly considered a safe manner.

I hereby release, hold harmless, and promise not to sue StreetLife Promotions and all event sponsors and officers, owners, directors, employees, or agents (collectively releases) with respect to any and all bodily injury and/or other loss resulting from my participation in the (title of event), whether as passenger or operator of a (motorcycle, bicycle or automobile), whether caused by the negligence of the releases or otherwise during any respect associated with riding or driving upon my own or my rental (motorcycle, bicycle or automobile).  In addition, by accepting of payment of a deposit, and/or my signature, whether electronic or otherwise, on the (title of event) registration form, I certify that I do not have any physical or mental condition or disability that would create a hazard for my fellow ride or driver participants or me.  I certify that I am over 18 years of age and am in possession of a valid (motorcycle or autombile) operator's permit endorsement on my valid driver's license. I, the ride or drive participant, be it (motorcycle or automobile) operator or passenger, I have read StreetLife Promotions Waiver & Release Agreement and will abide by all terms as stated herein. I hereby agree this waiver or release shall be binding upon and enforceable against me, my spouse, heirs, assigns, or personal representatives without limitation.

I have read and understand the terms of the release and waiver of responsibility I am about to sign and I further understand that StreetLife Promotions cannot accept any (title of event) participant, be it (motorcyle, bicycle or automobile) operator or passenger, without a duly signed release and waiver form. I understand clearly that I have given up substantial rights by signing this release and wavier of responsibility and have signed it freely and voluntarily without any inducement, assurance, or guarantee being made to me and intend my signature on or electronic submission of the (title of event) registration application to be a complete and unconditional release of all liability to the greatest extent allowed by law.

By signing and submitting this agreement and any other required forms, I acknowledge that I have met or exceeded any (motorcycle, bicycle or automobile) operation requirements imposed under the laws of (country).
